At this year's AECOS Congress in London, Chief Eye Surgeon Pavel Stodůlka won first place in the international video competition in the category Special Interest in Cataract Surgery & IOL Implantation. The award-winning video, titled "Lens will tell us the truth" impressed not only with its technical quality, but above all with its original approach to precise capsulotomy centering.

The video depicts the procedure on a patient with the rare Adie syndrome. It turned out that the commonly used method of centering the capsulotomy according to the dilated pupil did not work reliably here - it led to significant decentration. The solution turned out to be to focus on the trypan blue mark, i.e. the stained centre of the anterior lens capsule, which is created when it is stained with trypan blue.

This method, developed by Dr. Stodulka and his team, has shown significantly higher accuracy than the method based on pupil dilation.

In the award-winning video, a capsulaser was used, placed directly under the operating microscope, which made the capsulotomy very fast. Despite this, the capsulotomy remained decentered. Therefore, a capsular ring was implanted during the operation to help stabilize and center the capsular bag; even after the subsequent implantation of the ring, significant decentration is noticeable.

It was only after intraocular lens implantation that it became apparent that the capsulotomy was accurately centered with the lens optics - precisely because of the orientation according to the trypan blue mark.
The result after six weeks was excellent: the patient achieved uncorrected visual acuity of 1.0 for distance and J1 for near, and was highly satisfied with the outcome.

This video shows that the trypan blue mark centering method can significantly improve the accuracy of cataract surgery.
